We hope you find these informative packs useful. We’ve collated them from publicly available data that relates to prevalence rates, clinical management indicators, lifestyle habits and take up of support, and healthcare usage costs. These data packs were compiled as part of a programme within the Long Term Conditions (LTCs) Network of West Midlands Academic Health Science Network (WMAHSN) to help STP leads across health and social care, clinicians, managers and the public to better realise where substantive efforts focused on prevention should result in improved health for the local population. That is, to help people redress adverse lifestyle habits, to prevent LTCs occurring as far as possible, prevent deterioration of LTCs and empower patients to take responsibility for their care and condition.
